position is full time, 40 hours per week and Monday - Friday. This
Nurse will work (4) 10-hour shifts. Regular shift time is 8a-6:30p
and (1) day per week it is 10a-8:30p rotating. Off day would rotate
weekly.Under the supervision of a provider or registered nurse,
participates in the assessment, planning, implementation, and
evaluation of care. Performs nursing process while providing direct
patient care in a safe and effective manner that promotes quality
patient outcomes. Functions as a member of the interdisciplinary
healthcare team. If working in non-provider based, physician
practice sites, utilizing the medical model of care delivery,
patient assessments, plan of care, and discharge plan are performed
by a physician, nurse practitioner (NP), or physician assistant
(PA). Under this model, the LPN assists the provider in providing
prescribed care or treatment to the patient.Experience

- Completion of an accredited Licensed Practical Nurse (LPN)

- Assists licensed professional personal in performing nursing
process across the continuum to deliver age and developmentally
appropriate patient-family-centered care.

- Utilizes clinical decision support tools and resources to
recognize issues and prioritize care
- Utilizes appropriate documentation tools to record, measure,
and retrieve healthcare data.
- Provides patient and family education appropriate for

- Utilizes resources to plan and provide nursing care that is
safe, effective, and financially responsible.Physician Practice
Specifics
- Communicates with home health agencies, third-party payor, and
other community resources as needed to coordinate needs.
- Facilitates and provides ongoing communication with
patient/family, interdisciplinary staff and third parties (home
health, third party payers, pharmacies, and other resources) to
identify and resolve potential barriers.
- Performs outpatient and clinic care coordination and monitors
patients' care between clinic and home.
- Facilitates care via telehealth to assist with patient needs
and concerns
- Participates in providing discharge instructions as delegated
by licensed professional practitioners.Development (Fostering

- Completes orientation and initial assessment of competency
successfully by determined due date.

Note: The LPN may not perform the following tasks:
- Documentation of the initial inpatient admission
assessment.
- Initiation of Caregiver Protocols, with the exception of
influenza vaccine protocols.
- Initiation of the administration of blood products,
unsupervised.
- Initiate chemotherapy infusions or administer IV push
chemotherapy.
- Complete the initial assessment for restraints.
- Unsupervised administration of code resuscitation drugs by IV
