We go beyond traditional care by offering holistic health services and free transportation for patients, ensuring access and continuity of care. The Role Patient Load: 18 patients/day; max panel size of 450 patients Support: Dedicated team including your own MA and scheduler. Patient Population: Seniors with multiple chronic conditions, managed through a value-based care model . Additional perks: door-to-door patient transportation, in-house specialists (Cardiology, Podiatry, etc.), and holistic health services (Acupuncture, Tai Chi, Yoga, Nutrition). Compensation: $260K–$265K base annual performance bonus (up to $20K in year one, increases with partnership) Partnership: No buy-in required. Eligible within 12–18 months; 65% of physicians achieve partner status within a year. Partners typically earn $300K.
