Must be able to provide case logs Case logs must be from past 24 months. No specific amount;
except for special privileges like Intubation, Arterial Lines,
Thoracentesis, Paracentesis, CVC, Hemodialysis, Chest Tube, which
requires at least 10 cases in the past 24 months or director
supervision for at least the first 10 procedures.
